How complex systems fail
This is a summary of (some) knowledge of Richard I. Cook, who obtained this knowledge through his study of and experience in healthcare. I imagine a complex system is the ‘hospital’, or ‘healthcare’, or ‘healthcare plan’.
These ‘rules’ are not about failure, nor about healthcare. My place of employment fits very well. Now I’m going to brood on how I can use this information in how I deal with this particular complex system: where can I be most effective and use my skills best?
